Dance Champion Candidate Biography Questionnaire

Each year, the Boston Dance Alliance honors a dance champion at its spring benefit. The selection committee uses the following criteria: the nominee must have had great impact in the Boston Metropolitan area, provided a continuing legacy in terms of leadership, initiative, creativity and contribution (this can be through an institution, students, writings, dancers, choreography) and be at least 60 years old. This questionnaire is intended to assist the selection committee in identifying and analyzing qualified candidates. Please limit responses to space provided.
